Had an overnight stay in a Walsall Hospital Bed in the last year?

Healthwatch wants to hear about people’s experiences of leaving hospital.

Your feedback can help the NHS understand if everyone leaving hospital is getting the support they need.

We want to hear from:
• people who have been discharged from an NHS hospital in England in the last 12 months
• carers, relatives or friends answering about someone they support

We are interested in experiences where being discharged (leaving hospital) was delayed or difficult.

This might include when:
• The discharge was delayed by at least a day
• The discharge happened before they felt ready
• care, support, equipment, medication, transport or follow-up was not in place
• information was unclear

Why we are doing this.
We want to understand how delays in being discharged from hospital affect people, and whether people are leaving hospital safely and with the right support.

What you tell us will help Healthwatch England raise patient and carer experiences with national decision-makers, including the NHS and the Government.

What taking part involves
You will be asked to complete a short survey. We estimate it will take around 10-15 minutes.

The survey asks about:
• what happened when you, or the person you support, during the hospital discharge
• whether the right support was in place
• whether information was clear
• how the experience affected you
• what could have made things better
• We will also ask some questions about you, such as age, ethnicity, disability and where you live.

This helps us understand whether different groups have different experiences. You do not have to answer any question you do not want to.

If you are answering for someone else?
If you are a carer, relative or friend, please answer about the person who left hospital wherever you can.

If you do not know the answer, please choose “Don’t know / can’t remember”. There are some questions about your experience as a carer / friend / relative at the end.

How we will use your answers
We will use the survey findings in reports, briefings and other materials about hospital discharge. Your stories can help.

We may use anonymous quotes from survey responses, but we will remove details that could identify you.

We will NOT publish your name or contact details without asking you first.

Your information
Your answers will be stored securely. Healthwatch England staff working on this project will have access to the survey data. Healthwatch Walsall staff from your area will also have access to your answers for their local research.

At the end of the survey, you can choose whether you are happy for us to contact you about your experience. This is optional.

We will keep the information securely and only for as long as needed for this project and related reporting. We will handle your information in line with Healthwatch England’s privacy policy.
